The following are our 2009-2010 forms and Soccer Maine Handbook. The handbook includes important information and we encourage all our coaches and team administrators to be familiar with the information in the handbook.

Player Registration:

All MMFC players are selected at tryouts held in the Fall and Spring each year. All players will be required to register with the club, and pay the required fees, before they begin playing.

If you signed a Letter of Intent to be a rostered player or received written communication from your coach or team administrator that you were selected as a pool or training player for a MMFC team, please complete the on-line registration below.

Note: This on-line registration link only applies to players who have been selected to play on a MMFC premier team (either as a rostered or pool/training player, and for players registering for our U-10 and U-11 Academy Program)

MMFC CREDIT CARD AND CHECK PAYMENT POLICY 2009-2010

Payments are due as follow:

A deposit of $200 is due within 15 calendar days from the date you sign your letter of intent, but no later than October 1st for all registered players.
Each player's tuition balance is due in three equal payments on November 1st, December 1st, and March 1^st (credit card payments will be made on a reoccurring basis. Parents may make payments by credit card or by check. If payment is made by credit card, when the initial $200 payment is made, the credit card company (Authorize.net) will request that the card holder authorize MMFC to charge the balance of the players tuition in three equal payments on the dates noted above.
Pool /Training players will be required to pay in two installments - A deposit of $200 is due on October 1st, and the tuition balance is due on November 1st.
Academy Players will be required to pay in two installments - A deposit of $200 is due on November 1st, and the tuition balance is due on December 1st.

If payments are being made by check, they MUST be made on or before the required dates - no exceptions - a $5 late fee will be assessed for each day the payment is overdue).

Any overpayments will be reviewed by the Treasurer and Executive Director and refunds due will be made within 10 business days of such overpayment.

Note: If you did not complete our Registration form and medical release at tryouts, please download the forms below, complete it, sign the commitment section, and mail it in to the club as soon as possible. The club address is listed at the bottom of this page.

Risk Management Registration - all MMFC board members, coaches, team administrators, skills trainer, instructors must complete the new Soccer Maine Risk Management registration.

Referee Reimbursement:

Team Cash Reimbursement Form - reimbursement for team friendlies (sharing field rental cost). If you cannot submit on-line, please fill in the form on-line, and save the completed form in a pdf, and send back to MMFC Admin or print and mail.
